Samsun Urinary Incontinence Surgery (TOT and TVT)
Suspension (sling) surgeries supporting the urethra in stress-type urinary incontinence; Differences in TOT and TVT methods, process and recovery period.
When does the incontinence surgery come up?
Stress type in urinary incontinence; Surgical options are assessed if adequate response is not received from pelvic floor exercises, life arrangements and non-surgical practices, or if the complaint is advanced from the start.
The type of incontinence is established before deciding on surgery. Sling procedures are not appropriate for urge incontinence; this distinction is made through an examination and any tests considered necessary.
What Is Sling Surgery?
In suspension surgeries, a thin strip is placed under the middle section of the urinary tract, which aims to prevent urinary escape by supporting the duct in times of increased intra-abdominal pressure.
Two basic methods have been described according to the passageway of the strip within the body: TOT and TVT. The examination findings of which method is appropriate are determined based on previous surgeries and accompanying conditions.
TOT (Transobturator Tape)
In the TOT method, the strip is placed through an opening called the obturator in the thigh interior. Since this pathway is far from the abdominal cavity, it presents a different profile in terms of the risk of bladder and intra-abdominal organ injury.
In the first days after surgery, a feeling of tension or pain can be described on the thigh's inner face; this sensation usually recedes shortly.
TVT (Transvaginal Tape)
In the TVT method, the strip is placed following the retropubic pathway through the back of the pubic bone. This method is a technique that has been practiced for a long time and has extensive experience on it.
During placement, the bladder may need to be displayed. Method selection; The nature of the incontinence, the urethra function and the general condition of the person are assessed together.
Surgery process
Surgery is usually performed under spinal or general anesthesia and is short-lived, with the same day or one night's rest sufficient in most people.
A short-term probe can be used to monitor urination after surgery, making sure you can make your urine comfortable before discharge.
Recovery period
In the first weeks, it is asked to stay away from behaviors that increase intra-abdominal pressure. General recommendations:
Avoiding heavy lifting and challenging exercise for the specified time
Regulating nutrition and fluid intake to prevent constipation
Waiting for the time specified by your physician for sexual intercourse
Controlling factors that lead to smoking and chronic coughing
Do not interfere with the inspections
Risks and expectation
As with every surgical procedure, there are possibilities such as infection, bleeding, difficulty urinating, tissue problems attached to the sherry and continued complaint. These titles are discussed in detail in the pre-operative consultations.
Surgical results vary from person to person; no definitive and permanent outcome can be promised. The surgical decision is made together after the information is received.
