Data controller

In accordance with the Law on Protection of Personal Data No. 6698 (KVKK), your personal data is processed in the scope described below by Prof. Dr. Aşkı Ellibeş Kaya as the data controller.

Personal data processed

Only the data you transmit through this website is processed:

• Identity data: name and surname

• Contact data: phone number, e-mail address if available

• Demand data: the title you want to discuss, your preferred time range, and notes you want to add

• Process security data: technical data collected through cookies (cookie Policy for detail)

Specially qualified data

Health data is special-qualified personal data under the KVKK and is subject to a stricter protection regime. You do not need to share details about your health status when creating an appointment request; It is sufficient that you share this information during the examination.

If you prefer to write information about your health status in the free text field, we will process this data based on your explicit consent only for the purpose of evaluating your appointment request.

Processing purposes and legal cause

Your personal data; The receipt and evaluation of your appointment request is processed for the purposes of contacting you and planning health care.

The legal reason for the processing is your explicit consent under KVKK Art. 5/1. In terms of health data, the provisions of KVKK m.6 apply.

The sending of an information message is subject to a separate and optional approval; failure to provide this consent does not prevent your appointment request from being evaluated.

Transfer of data

Your personal data is not shared with third parties except for legal obligations; It is in no way transferred or sold to third parties for marketing purposes.

The appointment form does not store the information you enter on the servers of this site. Your request is forwarded to us through the messaging app on your own device. In this case, the relevant application's own privacy policy also applies.

Storage period

Your personal data is retained for as long as necessary for the purpose for which they are processed and for the mandatory retention periods stipulated in the relevant legislation. In the event of expiration, data is deleted, destroyed or anonymized.

Your rights as the person concerned

Pursuant to Art. 11 of the KVKK, you have the following rights:

• Learning whether your personal data is processed and requesting information on it

• Learn if it is used in accordance with the purpose and purpose of processing

• Knowing third parties to whom data is transferred at home or abroad

• Don't ask to be corrected if it is missing or misprocessed

• Do not ask that it be deleted or destroyed under the terms of the law

• Requesting notification to third parties where correction, deletion and destruction transactions are transferred

• Objection to the occurrence of a result against you by analyzing it with exclusive automated systems

• Do not claim damages if you suffer damages due to unlawful processing

• Don't take back the explicit consent you gave when you wanted it.

You can submit your requests for your rights via the email address or office address on the Contact page. Your applications will be finalized as soon as possible and within thirty days at the latest, depending on the nature of the request.
